West Bay 5th Grade Graduation Celebration 

TO DO (By thursday, march 13th)

To help cover the cost of the 5th grade graduation activities the last week of school, the celebration at Top Golf and the class t-shirt, please contribute $25 per student.  If your child is unable to attend the Top Golf Party, contribute $10.

• Complete student name and select teacher

• Select shirt size

• Complete parent name(s) in case of emergency
  

• Complete Cell Phone Number(s) in case of emergency
  

• List any Food Allergies (all students that attend Top Golf party will receive a turkey sandwich, chips, cookie and drinks)

Have any fun photo memories (e.g. First Day of Kindergarten, Fun Day, Fall Social, Art Show, Covid Home school) from Kindergarten-5th Grade, please send them to Amy Snodgrass at snodgrassamy@gmail.com.  Pictures will be used in the video shown during the graduation ceremony.
