West Bay 5th Grade Graduation Celebration 

TO DO (By Friday, March 1st)

To help cover the cost of the 5th grade graduation activities the last week of school, the celebration at Top Golf and the t-shirt, please contribute $25 per student.  Pay $10 if your student is unable to attend the all class Top Golf Party.

• Complete student name and teacher

• Complete parent name(s) in case of emergency
  

• Complete Cell Phone Number(s) in case of emergency
  

• List any Food Allergies (all students that attend Top Golf party will receive a sandwich meal and drinks)

Email Jenny Boll: jenniferjboll@gmail.com your student’s Kindergarten SCHOOL picture ONLY IF your student did not have a picture in the 2018-2019 West Bay yearbook. Pictures will be used in the video shown during the graduation ceremony.
